(5'alpha, 10alpha) -9, 10-dihydro-12'-hydroxy-2'-methyl-5 '- (phenylmethyl) ergotaman-3', 6 ', 18-trion. Most often, the medicine is synthesized in the form of a mesylate.

Chemical properties

Alpha-blocker Dihydroergotamine is a solution without color and odor. In chemical structure, it is a hydrogenated derivative of ergot alkaloids.

The substance is released in the form of tablets, a solution for oral administration and injection, as part of a nasal spray (inhalation).

Vasodilating, anti-migraine.

Pharmacodynamics and pharmacokinetics

Substance stimulates serotonin 5-HT1D receptors and blocks alpha2- and alpha1-adrenergic receptors in the vessels. Dihydroergotamine is a vasoconstrictor of intracranial vessels, exhibits pronounced neuroinhibitory properties. The tool eliminates the manifestations of phono- and photophobia. Under its action, the tone of the veins increases.

When using a nasal spray, inhalation, the drug quickly penetrates into the blood, due to this, the relief of an attack migraine happens fast enough within half an hour. The medicine is also excreted from the body quickly. The use of tablets reduces the bioavailability of the active substance. Intramuscular administration of the drug leads to the development of an anti-migraine effect after 15-30 minutes, the effect persists for 3-4 hours.

Indications for use

Dihydroergotamine is prescribed:

  • for stopping attacks migraine;
  • with vasospastic angina pectoris;
  • patients with arteriohypotonic headache;
  • at orthostatic hypotension;
  • at Raynaud's disease.


The medicine is not used:

  • at coronary heart disease;
  • at myocardial infarction;
  • in patients with severe atherosclerosis;
  • with organic heart lesions;
  • for the treatment of patients with uncontrolled arterial hypertension;
  • at sepsisobliterating vascular diseases;
  • in patients with renal and liver failure;
  • for the treatment of pregnant women;
  • at allergies on the active substance;
  • with lactation;
  • in old age.

Side effects

Dihydroergotamine may cause the following side effects:

  • diarrheanausea dizzinessvomiting
  • shortness of breath through the nose cardiac arrhythmia, drowsiness;
  • swelling, itching, and skin rashes;
  • paresthesia of limbs, spasm of peripheral vessels, collapse (seldom).

Dihydroergotamine, instructions for use (Method and dosage)

The dosage and method of use of the drug depends on the dosage form.

Dihydroergotamine is prescribed subcutaneously, intramuscularly, orally, in the form of inhalation.

With migraine, taking 5-20 drops of a 0.2% solution of the drug or 2.5 mg (1 tablet) is indicated; intramuscular or subcutaneous injection of 0.25-0.5 ml of a 0.1% solution once or twice a day.

In severe migraine permissible use of the drug in a dosage of 1 ml intramuscularly.

Injections, if necessary, can be repeated every hour or every 3 hours. You can use the nasal spray for a maximum of 4 times in each nostril.

Overdose may cause symptoms ergotism. This is manifested in respiratory depression; chest pains, general weakness, muscle weakness, cyanosisspasm of vessels located on the periphery, ischemia of the toes and hands, diarrhea, deliriumvomiting cramps coma (seldom).

Dihydroergotamine is not recommended to be combined with erythromycin, oleandomycin, doxycycline, tetracycline, dopaminealpha adrenergic blockers, nitroglycerin, vasodilators, beta-adrenergic agonists. This can lead to increased effects of the drug.

Combined technique with vasopressin and alpha-adrenomimetics weakens the effect of the drug.

Use of substance with reserpine may lead to a mutual reinforcement of the effects of the drugs.

Dihydroergotamine is used with extreme caution to treat patients under the age of 16 and after 65 years.

With complex treatment migraine and the use of other drugs, this substance is recommended to be administered parenterally.

During pregnancy and lactation

The medicine is not prescribed during pregnancy. The drug is excreted in breast milk.
